How many people take the entire dose at once for high carb meals?

I just had a cookie with 124g of carbs, my pump suggested 18u of insulin. I feel super uncomfortable taking that much at once so I extend it over 2 hours.

Depends on what’s in the meal.

Predominantly carb - up front (pre bolus!!), carb and protein moderate fat - up front sometimes split 80/20, high carb and high fat - extended 50/50 over a couple hours.

I don’t really do extended boluses, I find my highs come pretty quickly.

I used to do 80% up front and than 20% 2 hours later. But now I take it all at once.

I’ve never split my shots up. I always take my full amount, even if it’s 24 units fast acting I’ll take it.

I know my body well enough now to know how I’m going to react, and my regimen is nearly perfect! It’s all about what works for you.

I do the whole bolus so I don’t go high.

I usually do 60% 30 mins before and the rest an hour later. I also don’t feel comfortable with boluses that are more than my usual.

Same thing happened to us the other day. 13 yo is TD1… she had a frozen cappuccino and a sandwich which totaled to 198 carbs -- with her CR she had to take 33 units (highest dose she’s ever taken at once).

We were both nervous about it, but we know her ratios are correct so dosed the whole 33. I watched her Dexcom like a hawk for the next two hours! She spiked a little and then leveled out just fine.

Know and trust yourself. If you feel you’re going to drop low, do half and half.

I take it all at once. Although if it’s a high fat meal, like thanksgiving dinner, I will split the dose to accommodate for that.

We do half up front and half an hour after for my 14yo daughter but only if it’s a high fat content meal. It helps curb the spike and levels her sugars out faster.

It depends on what it is for me. Like pizza and Chinese food have a delayed spike for me, so I do extend it out most of the time.

But sweets like cookies, muffins, cake, etc hit me pretty fast, so I take it all ahead of time and usually wait to let it start working before I eat it. But I’ve never had a cookie with 124 carbs either though, so that may make a difference. But if I waited an hour or two for part of the dose for something like that, I would be sky high. We are all different though and just learn what works for us.

I would split 60/40 and extend 2 hours. You can always cancel it if the first part does the trick. I would still keep a close eye on my numbers though, that many carbs may blow you sky high in 30 minutes.

Then I would cancel the extended, and start correcting a little at a time to try and keep it out of DKA range.

What cookie has 124g of carbs in it!? That's a very large dose for cookies unless your sugar was already high to begin with.
